Submission Portal for New Patients

Please submit your information, including your name, date of birth, address, and telephone number, below to be contacted regarding new patient appointment availability.

To prevent misunderstanding, we must confirm directly with the person who is to be seen that they wish to attend the appointment.

We will do our best to inform you via email if we are temporarily closed to new appointments.

Due to demand, it is possible an appointment will not be available in a period of time that is convenient for you. Please continue checking with your other resources while we work on your scheduling.

When using insurance benefits, please be prepared to email an image of your insurance card (front and back), so we can ensure that provided services are covered for you.

If we cannot confirm your insurance validity before the appointment, payment in full will be expected at time of service.

Insurances accepted: Aetna, Blue Cross Blue Shield, Covenant Management Systems, Golden Rule, Oscar, and UnitedHealthcare.

Insurances no longer accepted: Ambetter, Cigna, Humana, Medicaid, Medicare, Sendero, Seton Healthcare, and Tricare.

Due to contract restrictions, we cannot provide "self pay" services to new patients with Medicare or Tricare.
